Lähtökohdat
Juurihoidon vaativuutta voidaan arvioida hampaan juurihoitoa käsittelevässä Käypä hoito -suosituksessa esitetyllä juurihoidon vaativuuden arviointilomakkeella (JH-VA).
Tutkimuksen tavoitteena oli selvittää lomakkeen käyttökelpoisuutta hammaslääketieteen opiskelijan näkökulmasta sekä kartoittaa, kuinka hyvin lomakkeen avulla tehty arvio vastasi hoidon toteutusta ja lopputulosta.
Menetelmät
Opiskelijoiden ja opettajan tekemiä juurihoidon vaativuuden arviointeja verrattiin toisiinsa tilastollisesti. Hoidon tulosta ja sitä, miten opiskelijat kokivat JH-VA:n käytön, selvitettiin erillisillä kyselyillä.
Tulokset
Opiskelija ja opettaja päätyivät erilaiseen arvioon juurihoidon vaativuudesta yli puolessa (55 %) tapauksista. Näistä suurimmassa osassa (71 %) opiskelijat olivat arvioineet juurihoidon helpommaksi kuin opettaja.
Tietyt potilaaseen liittyvät tekijät, normaalista poikkeava kruunumorfologia ja komplikaatiot aiemmassa juurihoidossa olivat merkitsevästi yhteydessä havaittuihin komplikaatioihin.
Opiskelijoiden mukaan JH-VA oli helppokäyttöinen, vaikkei sen avulla selvinnytkään heidän oma kompetenssinsa tehdä kyseinen juurihoito.
Johtopäätökset
Yleensä opiskelijat arvioivat hoidot helpommiksi kuin mitä ne todella olivat. Tulokset antavat mahdollisuuden kehittää JH-VA-lomaketta sopivammaksi opetuskäyttöön.

Usefulness of an endodontic case assessment form in dental education
The objective of our study was to explore the usefulness of an endodontic case assessment form (ECAF), presented in the Finnish Guidelines for Endodontic Treatment, according to fifth year dental students. We also wanted to explore how well the outcome by the ECAF form corresponded with the root canal treatment process and its outcome (Golden standard by the senior clinical teacher).
Students’ and teacher’s assessments of the root canal treatment were compared statistically when the students filled the ECAF at baseline of a root canal treatment and the teacher filled a teacher’s assessment form after the treatment. The outcome and the students’ experiences of the use of ECAF were explored by separate forms.
The assessment outcome on the difficulty of a root canal treatment by the student and the teacher were different in more than half of the cases. In the majority of these the students had evaluated the case to be easier than the teacher. The greatest difference between the assessments of the student and the teacher appeared to be in the moderately difficult cases. Patient derived factors, especially gagging, deviant crown morphology and complications in the previous endodontic treatment, were all significantly related to observed complications. The students found the ECAF form to be user friendly, even if it did not demonstrate their competence to treat the specific case.
The endodontic case assessment form (ECAF) appears to be most useful in specifically distinguishing the least and most difficult cases. Dental students generally estimate the cases to be too easy. A simpler form, including all the discovered factors, might be more useful for dental students.
